Ohio Home Care Waiver

The Ohio Home Care Waiver meets the home care needs of individuals up to age 60, whose medical condition would otherwise require them to live in a nursing home or other institution. The Ohio Department of Job and Family Services, Bureau of Home & Community Services administers this program.

Case management services are contracted to a vendor to assess eligibility with regard to level of care needed. Region Four vendor for Lorain County is:
