Case Management works with individuals and families who are struggling with multiple barriers to getting SSI and/or Medicaid.  Clients are assigned a case manager to assess needs and advocate for them to receive SSI benefits.  Case managers assist clients with referrals and transportation arrangements to doctors' appointments.  Regular contact with these clients is maintained to ascertain that needed services and documentation needed to receive SSI or Medicaid services are obtained.  In addition, case managers provide outreach at Lorain County Health and Dentistry.
