Please Note: You only need ONE application for multiple programs.
For example, if you are applying for Cash, SNAP, Childcare and Medical Assistance, only complete one application online or on paper, be sure to check all boxes you wish to apply for.
How to apply for OWF Cash Assistance
- Apply online using the following link: https://benefits.ohio.gov/
- Apply over the phone using Shared Services at 1-844-640-6446. Please see below for list of the prompts.
- Print out a JFS 7200 Application for Assistance and submit it via U.S. Mail, Fax (440-323-3422), or e-mail ([email protected])
- Complete an application in the LCJFS lobby and submit it to the front desk

How to apply for SNAP

- Print out a ODM 7216 Application for Health Coverage & Help Paying Costs and submit ot via U.S. Mail, Fax (440-323-3422), or e-mail ([email protected])
- Complete an application in the LCJFS front lobby and submit it to the front desk
How to apply for Waiver
- Print out a ODM 2399 Request for Medicaid Home and Community-Based Services and a JFS 7200 Application for Assistance and submit them via U.S. Mail, Fax (440-323-3422), or e-mail ([email protected])
- Complete the ODM 2399 and JFS 7200 Application for Assistance in the LCJFS lobby and submit them to the front desk
