How to Apply
Please Note: You only need ONE application for multiple programs.
For example, if you are applying for Cash, SNAP, Childcare and Medical Assistance, only complete one application online or on paper, be sure to check all boxes you wish to apply for.
How to apply for SNAP, OWF Cash Assistance, Child Care, or Medicaid
- Apply online using the following link: https://benefits.ohio.gov/ or https://www.lcdjfs.com/forms
- Apply over the phone using Shared Services at 1-844-640-6446. Please see below for list of the prompts.
- Print out a JFS 7200 Application for Assistance and submit it via U.S. Mail, Fax (440-323-3422), or drop box. For an existing case, you can upload your application using our Document Upload feature.
- For Medicaid only you may also print out a ODM 7216 Application for Health Coverage & Help Paying Costs and submit via U.S. Mail, Fax (440-323-3422), or drop box. For an existing case, you can upload your application using our Document Upload feature.
- Complete an application in the LCJFS lobby and submit it to the front desk

How to apply for Waiver
- Print out a ODM 2399 Request for Medicaid Home and Community-Based Services and JFS 7200 Application for Assistance and submit them via U.S. Mail, Fax (440-323-3422), drop box outside the agency, or submit them to the front desk.
- Complete the ODM 2399 and JFS 7200 Application for Assistance in the LCJFS lobby and submit them to the front desk
